Change to Register
Particulars on the Register
The Board is required to maintain a Register in accordance with section 151 of the Health Professionals Regulation. The Board is responsible to ensure that the information in the Register is as accurate and current as practicable.
The Register must contain the registration details of each person registered by the Board and include:
a. the person’s name;
b. the name used by the person when practising the profession if different from the person’s name;
c. any other name previously used by the person in the practise of medicine;
d. the person’s date of birth;
e. the person’s postal address;
f. the person’s business address or intended business address;
g. evidence of the person’s qualifications, including relevant training undertaken;
h. if the person has been refused registration, or has had his or her registration cancelled - details of the refusal or cancellation.
i. if the person has been refused registration in another jurisdiction, or has had his or her registration cancelled in another jurisdiction - details of the refusal or cancellation;
j. the provision under which the person was registered;
k. the person’s registration number;
l. the registration date and registration end date;
m. any conditions of registration; and
