APPLICATION FORMS

Medical Board Of The Australian Capital Territory

The Medical Board of the ACT uses two application forms:

Mutual Recognition Application Form

A medical practitioner who holds current registration in another Australian State or Territory is able to apply for registration in the ACT by lodging an application using the Mutual Recognition Application Form.

Use this form to apply for:

Initial Registration Application Form

Those practitioners who are not currently registered elsewhere in Australia may apply for registration in the ACT by completing the Initial Registration Application Form.

Use this form to apply for:

·        Registration as an Intern (click here)

·        Registration as an AMC Trainee - Competent Authority (click here) - Standard Pathway (click here)  

·        Registration as a Post Graduate Trainee (click here)

·        Registration as a Specialist (click here)

·        Registration as an Australian or New Zealand medical graduate (click here);

Changing from General to Specialist Registration

Practitioners are required to lodge a written application with the Board (click here.  The application is to be accompanied by a certified copy of the specialist qualification and the determined fee, currently $50.  Qualifications recognised by the Board may be found at (click here).

 

Applications must be approved by the Board before the applicant can commence specialist medical practice.
